摘要
In the immunocompromised setting, there are distinct radiologic findings of primary central nervous system lymphoma (PCNSL), including necrotic ring-enhancing lesions, increased propensity for intralesional haemorrhage, and multiplicity. In this clinical context, advanced imaging with MR perfusion, spectroscopy, and diffusion-weighted imaging can be used to increase accuracy in the diagnosis of lymphoma over mimics such as high-grade glioma, metastases, or infection. This review summarizes the histology and pathophysiology of PCNSL in immunodeficient hosts, which provide a basis for its imaging appearances, prognosis, and treatment. This discussion is important for the general radiologist as the incidence of immunodeficiency-related PCNSL may be increasing. Dans un contexte d'immunovuln & eacute;rabilit & eacute;, le lymphome primitif du syst & egrave;me nerveux central (LPSNC) pr & eacute;sente des caract & eacute;ristiques radiologiques distinctes, notamment des l & eacute;sions n & eacute;crotiques pr & eacute;sentant un rehaussement annulaire, une propension accrue aux h & eacute;morragies intral & eacute;sionnelles et la multiplicit & eacute; des l & eacute;sions. Dans ce contexte clinique, une imagerie avanc & eacute;e au moyen d'une IRM de perfusion, de la spectroscopie et de l'imagerie de diffusion peut & ecirc;tre utilis & eacute;e pour augmenter la pr & eacute;cision du diagnostic de lymphome par rapport aux affections de caract & eacute;ristiques radiologiques similaires telles que le gliome de haut grade, les m & eacute;tastases ou les infections. Cet article de revue r & eacute;sume l'histologie et la physiopathologie du LPSNC chez les h & ocirc;tes immunovuln & eacute;rables, qui fournissent une base pour l'imagerie, le pronostic et le traitement.
