摘要
This article reveals the importance of integrating different health specialists in integrated work with community leaders. This study was developed at the "30 de Noviembre" Teaching Polyclinic of the Santiago de Cuba municipality. Cuba. Qualitative methodology was used and action research was used as a method, which facilitates community work and the active participation of subjects in the research. The objective is proposed: to develop a system of actions that, through community leadership, contribute to participation in the promotion of health and the prevention of chronic non-communicable diseases. The main results are related to the preparation for joint work of leaders, specialists and patients. Socialization and communication in the community of assertive messages contribute to the health of subjects with high blood pressure.
