The Passot technique for oncoplastic breast surgery

BackgroundIn 1925 Passot described the transposition of the nipple-areola complex (NAC) with an inferior pedicle and repositioned through a circular incision in the upper skin flap, which allows a breast reduction without a vertical scar. To date there is no description of this technique for cases of oncoplastic breast surgery.MethodsAll patients operated on with oncological criteria and reconstructed using the lower pedicle for vascular support of the NAC without a vertical scar were reviewed and included in this study. The selected cases were patients with hypertrophic breast and the NAC located more than 30 cm from the sternal manubrium. The upper external and internal quadrants of the breast were removed under margin control.ResultsTwenty patients with breast carcinoma (ductal carcinoma in 14 patients - lobular carcinoma in 6 patients) operated on with oncological criteria and reconstructed using the lower pedicle for vascular support of the NAC without a vertical scar. In all cases, no necrosis of the NAC was observed. Reconstruction was performed with the lower quadrants, obtaining a very acceptable final aesthetic result. Implants were used in only one case. The final position of the NAC was 20 cm from the sternal manubrium.ConclusionsThe Passot technique herein presented for oncoplastic breast surgery in patients with breast carcinoma and breast hypertrophy allows for oncological resection, immediate reconstruction with autologous tissue and correction of breast ptosis.Level of Evidence: Level IV, therapeutic study.ConclusionsThe Passot technique herein presented for oncoplastic breast surgery in patients with breast carcinoma and breast hypertrophy allows for oncological resection, immediate reconstruction with autologous tissue and correction of breast ptosis.Level of Evidence: Level IV, therapeutic study.
