The Ovarian Adult Granulosa Cell Tumors: A Retrospective Clinicopathological and Survival Outcome Analysis

Purpose This study aims to analyze the clinicopathological factors of adult granulosa cell tumors (AGCT) and their impact on survival outcomes. Methods This is a retrospective observational study of 21 patients with histopathologically diagnosed AGCT from January 2010 to December 2019. Surgery was the mainstay of management with adjuvant chemotherapy administered as per the final FIGO staging. Survival analysis was done using the Kaplan-Meier method, and correlation of various prognostic factors was done. p-value of < 0.05 was considered significant. Results The mean age of presentation was 45 years with the majority belonging to the less than 50 years group (61.9%). Abdominal pain and postmenopausal bleeding were the major presenting symptoms (57.1%). Most cases had presented at stage I (66.7%). Stages II and III presentation were noted in 4.8% and 33.3%, respectively. The median follow-up was 87.5 months. Recurrence occurred in 23.8% of cases. A 5-year overall survival (OS) of 95.2% and disease-free survival (DFS) of 75.6% were observed. The stage of the tumor was found to be a significant factor affecting both OS and DFS. The presence of residual disease had a significant impact on DFS. Conclusion Granulosa cell tumors exhibit positive survival outcomes. Hence, identification of patients at an earlier stage and timely intervention with optimal debulking will help to mitigate the severity of the tumor providing a good quality of life.
