创伤患者血清IL-33和sST2的变化及意义

目的 研究创伤患者血清白细胞介素-33(IL-33)和可溶性ST2(sST2)的变化及其与炎症指标C-反应蛋白(C-reactive protein,CRP)和损伤程度评分(injury severity score,ISS)的相关性。方法 53例创伤患者分别于创伤后4 h、24 h、72 h、7 d等时间点抽取外周静脉血,分别检测血清IL-33、sST2、CPR水平,并与健康对照组比较;分析血清IL-33水平与sST2、CPR和ISS的相关性。结果 创伤后4~24 h患者血清IL-33和sST2水平分别较健康对照组明显升高(P<0.05),创伤后4 h7 d患者血清CRP水平较健康对照组明显升高(P<0.01);创伤后4 h7 d患者血清IL-33分别与sST2和CRP水平呈正相关(r=0.347、0.386,P<0.01),患者血清sST2与CRP水平呈正相关(r=0.276,P<0.01)。ISS评分分别与创伤后4 h和24 h患者血清IL-33水平呈正相关(r=0.308、0.433,P<0.05,P<0.01)。结论 创伤可导致IL-33和sST2释放,其释放水平与炎症反应程度和创伤程度相关。释放机制可能是创伤后细胞损伤和坏死所致。IL-33和sST2可能参与和调节创伤后炎症反应,在创伤后防御感染和促进创伤愈合方面起重要的作用。
