Sarcoidosis is a multisystemic disease characterized by the presence of non-caseating granulomas. We present a 26-year-old man who presents to the emergency department with fever, myalgia, excessive sweating, and weight loss. The analysis reveals pancytopenia, high than 150 U/L; splenomegaly, peri-aortic-caval retroperitoneal adenopathies confirmed by imaging. After ruling out the most common causes, a bronchoalveolar lavage showed an elevated CD4/CD8 ratio and lymphocytosis exceeding 50%, as well as confluent non-caseating epithelioid granulomas in the biopsy of the adenopathy, establishing the diagnosis of stage III sarcoidosis.
