In summary, despite relevant advances in the understanding of the pathophysiology portal hypertension, in clinical practice, the armamentarium is limited to beta -blockers for the long -term treatment and to terlipressin, somatostatin, and octreotide for the treatment of acute variceal bleeding. The recent demonstration of the benefit beta -blockers to prevent overall cirrhosis decompensation has introduced major changes in the clinical guidelines and has fostered a new interest in development drugs specifically to decrease portal pressure in cirrhosis.
