We report a case of a 43-year-old man with bilateral distal biceps tendon rupture while performing pull-ups undergoing a bilateral simultaneous surgical repair with a cortical suspension device couple with an interference screw. At 5-month follow-up each elbow regained full ROM and strength. The patient returned to the preinjury level of sport and participated with excellent results in gymnastic competitions.
