Radiological results and clinical outcomes of the Metaizeau technique for pediatric radial neck fractures A retrospective study on a reliable surgical option in children

BACKGROUND: Pediatric radial head fractures are uncommon, accounting for 1% to 2% of all pediatric fractures and 5% to 10% of elbow fractures. Fractures with minimal displacement can be treated conservatively. In the most advanced degrees of displacement, closed surgical reduction can be performed by the Metaizeau technique, consisting in the use of retrograde intramedullary K wires fixation. METHODS: From 2014 to 2019, six patients, aged between eight and twelve years old, underwent closed reduction surgery according to the Metaizeau technique and were evaluated with radiological, clinical, and functional assessments, such as elbow axis and motions examinations and Mayo Elbow Performance Score measurements; problems, complications and satisfaction were also reported and analyzed. RESULTS: After a mean 4 -year follow-up, no differences were found between elbow range of motion of the operated side in comparison to the contralateral side, such as for the measurements of elbow valgus angle; moreover, no cases of daily living and sporting activities limitations, problems, complications, or dissatisfaction were reported. CONCLUSIONS: Excellent and satisfactory radiological, clinical, and functional outcomes and results can be expected after the Metaizeau technique for the treatment of radial neck fractures in children. This data should encourage the validity and the reliability of this surgical practice for pediatric radial head and neck fractures.
