In summary, talus fractures are rare injuries and can cause significant morbidity and disability. They are a result of high-energy trauma, and many are associated with life threatening injuries. They can be difficult to diagnose due to their complexity, and a thorough evaluation is necessary which includes both clinical and radiographic examination. In most talus fractures, a CT scan is required to diagnose and aid in proper treatment. Most fractures require ORIF, and anatomic reduction is imperative in improving outcomes. Even with anatomic reduction and internal fixation, the risk of complications is high and includes posttraumatic arthritis, AVN, and malunion.
