The PAP flap, when harvested in the suprafascial, thin or superthin plane, provides a good source of thin pliable tissue for resurfacing of defects in the hand and upper extremity, with a concealed donor site. Preoperative flap planning using CTA or CDU is essential for obtaining information regarding the location and course of the dominant perforator.
