Preserving the larynx when attempting to reconstruct the hypopharynx is challenging. In some cases of extensive lesions of the upper digestive conduit, a combination of different techniques for reconstruction may be required. Here, we report the case of a teenager with an extensive esophagogastric lesion secondary to caustic ingestion. A larynx-preserving hypopharyngeal and esophageal reconstruction with colon interposition and ileal free flap was performed.
