Anterior spinal cord syndrome from a spinal epidural hematoma following removal of an epidural catheter

摘要
PurposeSpinal epidural hematoma (SEH) is a rare yet significant complication associated with neuraxial anesthesia. Here, we present the case of a 74-yr-old male who underwent open repair of an abdominal aortic aneurysm. Following the removal of an epidural catheter, the patient developed anterior spinal cord syndrome due to an SEH despite having a normal coagulation profile.Clinical featuresThis patient's neurologic presentation was marked by a loss of motor function while maintaining fine touch sensation distal to the spinal cord injury. Initial truncal computed tomography (CT) angiography failed to detect vascular compromise or diagnose the SEH. Subsequently, delayed magnetic resonance imaging (MRI) revealed a multilevel thoracic epidural hematoma, spinal cord infarction, and ischemia. Immediate surgical decompression was performed, but unfortunately, the patient had a poor outcome.ConclusionAnterior spinal cord syndrome (ASCS) represents an uncommon neurologic manifestation of SEH, which is typically characterized by a triad of back pain and sensory and motor deficits. Although the initial CT scan was necessary to diagnose the postvascular surgery complication, it did not immediately detect the SEH. In cases of ASCS subsequent to thoracic epidural placement and removal, MRI is the preferred imaging modality for precise diagnosis and assessment of the need for surgical intervention. Despite adherence to anticoagulation guidelines, patients undergoing neuraxial anesthesia may face an elevated risk of developing SEH. Health care professionals should remain vigilant in monitoring for neurologic abnormalities following epidural catheter insertion or removal, particularly in the context of vascular surgery. Nous pr & eacute;sentons ici le cas d'un homme de 74 ans qui a b & eacute;n & eacute;fici & eacute; d'une r & eacute;paration ouverte d'un an & eacute;vrisme de l'aorte abdominale. Apr & egrave;s le retrait d'un cath & eacute;ter p & eacute;ridural, le patient a d & eacute;velopp & eacute; un syndrome m & eacute;dullaire ant & eacute;rieur d & ucirc; & agrave; un h & eacute;matome p & eacute;ridural rachidien malgr & eacute; un profil de coagulation normal.Caract & eacute;ristiques cliniquesLa pr & eacute;sentation neurologique de ce patient & eacute;tait marqu & eacute;e par une perte de la fonction motrice tout en conservant une sensation de toucher fine distale & agrave; la l & eacute;sion m & eacute;dullaire. L'angiographie initiale par tomodensitom & eacute;trie (TDM) n'a pas permis de d & eacute;tecter d'atteinte vasculaire ni de diagnostiquer un h & eacute;matome p & eacute;ridural rachidien. Par la suite, une imagerie par r & eacute;sonance magn & eacute;tique (IRM) retard & eacute;e a r & eacute;v & eacute;l & eacute; un h & eacute;matome p & eacute;ridural thoracique & agrave; plusieurs niveaux, un infarctus m & eacute;dullaire et une isch & eacute;mie. Une d & eacute;compression chirurgicale imm & eacute;diate a & eacute;t & eacute; r & eacute;alis & eacute;e, mais malheureusement, l'issue a & eacute;t & eacute; mauvaise pour le patient.ConclusionLe syndrome m & eacute;dullaire ant & eacute;rieur repr & eacute;sente une manifestation neurologique peu fr & eacute;quente de l'h & eacute;matome p & eacute;ridural rachidien, qui se caract & eacute;rise g & eacute;n & eacute;ralement par une triade de maux de dos et de d & eacute;ficits sensoriels et moteurs. Bien que la tomodensitom & eacute;trie initiale ait & eacute;t & eacute; n & eacute;cessaire pour diagnostiquer la complication chirurgicale post-vasculaire, elle n'a pas imm & eacute;diatement d & eacute;tect & eacute; l'h & eacute;matome p & eacute;ridural rachidien. Dans les cas de syndromes m & eacute;dullaires ant & eacute;rieurs cons & eacute;cutifs & agrave; la pose et au retrait d'un cath & eacute;ter p & eacute;ridural thoracique, l'IRM est la modalit & eacute; d'imagerie privil & eacute;gi & eacute;e pour un diagnostic pr & eacute;cis et une & eacute;valuation de la n & eacute;cessit & eacute; d'une intervention chirurgicale. Malgr & eacute; le respect des directives d'anticoagulation, les patient<middle dot>es b & eacute;n & eacute;ficiant d'une anesth & eacute;sie neuraxiale peuvent faire face & agrave; un risque & eacute;lev & eacute; de d & eacute;velopper un h & eacute;matome p & eacute;ridural rachidien. Les professionnel<middle dot>les de la sant & eacute; doivent demeurer vigilant<middle dot>es dans le monitorage des anomalies neurologiques & agrave; la suite de l'insertion ou du retrait d'un cath & eacute;ter p & eacute;ridural, en particulier dans le contexte d'une chirurgie vasculaire.
