Background: The osteomeatal unit (OMU) aids sinus drainage. Cone beam computed tomography (CBCT) scans offer accurate data, isotropic voxel values, and fast three-dimensional (3-D) evaluation of the maxillofacial region. Objective: To assess the drainage system of the maxillary sinus (MS) and its relation to variations and pathologies using CBCT. Method: A retrospective evaluation of CBCT images from 134 sinuses was conducted, measuring the height of ostium and infundibulum length (IL). MS pathology and anatomical variations were recorded. Statistical analysis was performed. Results: Significant differences in ostium height (OH) and IL between male and female on both sides. IL in the maxillary sinuses showed significant associations with partial/total opacification. IL on the right side is related significantly to the presence of Haller cells (HCs), whereas on the left side, related to maxillary sinus septa (MSS). Conclusion: Drainage from MS is prone to anatomical changes and pathologies. CBCT radiographic examination offers a cost-effective and efficient diagnostic tool for assessing MS drainage systemss.
