Unruptured ovarian pregnancy: a rare case report

Background: Ectopic pregnancy (EP) refers to the implantation of a fertilized egg outside the uterine cavity, most often in the fallopian tube, while occurring in the ovarian tissue is rare, accounting for only 0.5% to 3% of all types of EP. The onset of ovarian pregnancy (OP) is acute, severe, and the fatality rate is high. Unruptured OP is extremely rare. OP lacks typical clinical manifestations, so how to improve the diagnosis rate is very important. Case Description: We report a rare case of unruptured OP of a 22-year-old woman, with a chief complaint of "amenorrhea for 48 days, slight vaginal bleeding for 1 day" to obstetrical department of Second West China Hospital of Sichuan University. Before surgery, we highly suspected OP based on ultrasound findings. Following thorough discussions with the patient, a timely laparoscopic removal while preserving the ovary was carried out. Finally, OP was confirmed based on intraoperative findings and pathological examinations. Conclusions: OP is a rare case without special features. Its diagnosis is so difficult and depends on criteria based on intraoperative findings and pathological examination. Laparoscopic surgery is considered the optimal treatment for OP, as gynecologists we need to strike a balance between remove pregnancy tissue and preserving normal ovarian tissue.
