The authors present the case report of a 76 years old woman with a giant aneurysm of the extracranial part of the internal carotid artery. An intraluminal shunt was not used during the surgical procedure. An autogenous saphenous vein interposition with aneurysm resection was performed. In the literature, aneurysms of the extracranial carotid artery are uncommon arterial lesions. For the high internal carotid artery aneurysm, extended incisions from the thyroid to the mastoid process may be helpful.
