When the degree of life and psychic conscience of the patient is not known, how can it be met as well as possible? On the basis of a clinical questioning of the carers in palliative care, this contribution tries to bring to light their questions. Being based on the clinical experiment of carers from other disciplines, a typology of the patients "with the unknown spirit" is proposed here while wondering about the ethical dimension of their being taken in charge. The suggested reasoning invites to wonder: "who is this patient I have to take care of?" It results some specific points of attention for a practice of the palliative care which would respect this type of patients as well as the professionals.
