A 15-year-old male patient with Behçet's disease responded to antiphlogistic therapy with cyclophosphamide and prednisone but not to prednisone alone. Studies of anaphylactic Arthus and delayed hypersensitivity during the course of therapy suggested that the inflammatory changes associated with disease activity are related more to delayed hypersensitivity than to anaphylactic or Arthus hypersensitivity. Measurement of delayed cutaneous hypersensitivity correlated better with the response to cyclophosphamide therapy in this patient than the decrease in the number of small circulating lymphocytes. The potential usefulness of cyclophosphamide in the treatment of Behçet's disease and other mucocutaneous syndromes is suggested by these studies. © 1969.
