摘要
Meeting the needs of people with chronic diseases is one of the major challenges facing U.S. health care today. Chronic disease is the leading cause of illness, disability, and death in the United States. It accounts for the majority of health care resources used each year and places substantial burdens on the quality of life of individuals, families, and communities. Successful chronic disease self-management is a partnership between an informed patient, the patient's family or caregiver, and the health professionals who care for the patient. Information available on the Internet can enable the patient to become an active partner in decision making and care.
