RELATION BETWEEN MICROVESSEL DENSITY, EXPRESSION OF KI-67 AND P53 WITH CLINICOPATHOLOGIC PARAMETERS AND PATIENT PROGNOSIS IN PROSTATE CANCER

Introduction: The aim of the study is to investigated the prognostic value of microvessel density ( MVD), Ki-67 staining intensity and existence of p53 mutation together with clinicopathologic parameters in patients who underwent radical retropubic prostatectomy (RRP) due to the localized prostate cancer. Materials and Methods: In our clinic, pathological specimens of 69 patients who had clinical and pathological data after RRP were retrospectively evaluated. The area with the greatest density of each of CD31, Factor VIII and CD105 positive cell clusters were chosen separately for counting MVD in X200 magnification. Greater than 5% of the tumor cells which demonstrated p53 nuclear reactivity were graded as positive. The percentage of nuclear Ki67 staining was grouped as negative, 1-10% and more than 10%. Results: Mean follow-up of the patients was 37 (17-106) months. Disease progression was determined in 17 (24.6%) patients during follow-up. Distribution of pathological stage in progressive patients was more advanced than non-progressive counterpart (p=0.01). Transitional zone PSA density (TZPSAD) of nonprogressive patients was statistically lower than progressive patients (p=0.01). Multivariate Cox regression analysis revealed pathologic stage, Gleason score (GS) and TZPSAD as independent predictors of progression free survival. Kaplan Meier analysis revealed that higher pathologic stage and GS significantly shortened the time to progression (p=0.003, p<0.001, respectively). Conclusion: MVD, percentage of Ki-67 staining and p53 protein expression do not add any predictive value in regard to progression after RRP compared with prognostic markers such as pathologic stage and GS.
