Cerebrospinal fluid ascites is a relatively rare complication that has been reported in the literature since 1972. Along with abdominal pseudocyst, cerebrospinal fluid ascites corresponds to 2% of the complications of ventriculoperitoneal shunts. Multiple theories have tried to explain the pathogenesis of this disease. Herein we report on a case of a 36-year-old male of the Hospital Mexico, who after 11 years of ventriculoperitoneal shunt placement developed cerebrospinal fluid ascites whose clinical studies showed no apparent underlying cause of this complication for what it was needed to perform a ventricular-atrial shunt. This report includes also a review of the existing literature.
