We report a case of ischemic stroke in a 38-year-old patient, related to a dissecting aneurysm of the carotid bulb, in which the angioscannographic aspect is evocative of an atypical focal dysplasia. This rare disease, described preferentially in black subjects, has never been reported in sub-Saharan Africa. Through a literature review, we recall the arguments of the clinical and radiological diagnosis. Its high potential for recurrence with medical treatment only and the proven effectiveness of surgical treatment therefore requires that it should be known and diagnosed by practitioners working in this geographical area.
