A Case of Pigmented Basal Cell Carcinoma: A Possible Dermoscopic Pitfall

A 67-year-old male patient was referred to our Dermoscopy Unit with a history of approximately 3 years of a brown papular lesion in the frontal region measuring 0.7x0.6 cm. On dermoscopy, it was difficult to distinguish maple leaf-like bulbous extensions from pseudopods, because they were not separate from the adjacent homogenous pigmented area. Since there were no other suggestive dermoscopic features of basal cell carcinoma, a differential diagnosis between pigmented Spitz nevi and melanoma could not be made. This case, in which the histopatological examination was the gold standard in the diagnosis, is presented as a possible dermoscopic pitfall.
