There is accumulating evidence that lipoproteins are an independent risk factor in the pathogenesis of glomerular injury leading of focal glomerulosclerosis in human and in experimental models of chronic renal disease. Experiments on glomerular mesangial and epithelial cells in culture underline the potential role of oxidized LDL in the development of glomerular injury and damage. Several lipid lowering drugs have been shown to prevent focal glomerular sclerosis or slower the progression of the disease in experimental settings using the model of the obese Zucker rat. Treatment with HMG-CoA reductase inhibitors have been also shown to be effective in human and first promising results seem to justify lipid lowering therapy. This review focuses on recent data that support the role of lipid mediated glomerular injury in the pathogenesis of progressive renal disease.
