A 60-year-old hypertensive man was incidentally diagnosed of a right sinus of Valsalva aneurysm measuring 7 cm and causing mild aortic regurgitation. Under cardiopulmonary bypass and aortic cross-clamping, the aneurysm was resected and the defect was reconstructed with a Dacron Hemashield Platinum (R) patch graft (Maquet, Rastatt, Germany). The right coronary ostium was reimplanted and a saphenous vein graft constructed to the first marginal branch (finding in preoperative coronary angiography). Intraoperative echocardiography showed trivial aortic regurgitation. As postoperative complication, the patient had a left middle cerebral artery stroke, with restitutio ad integrum.
