Objective: To report a case of Balantidium coli colitis in a German butcher. B. coli is an actively motile, ciliated protozoan preferentially inhabiting the large bowel. Design: Case report. Patient: A 55-year-old male German butcher with abdominal pain and a positive haemoccult test. Results: A colonoscopy revealed a single colonic ulcer in the caecal region. Histology revealed multiple trophozoites of B. coli in specimens obtained from the ulcer. The patient was successfully treated with terramycin. Conclusions: Although balantidiasis is a rare disease in Western Europe, it deserves attention because it can prove fatal in some patients.
