... the problem of designing institutions that do justice to the equal importance of all persons, without making unacceptable demands on individuals, has not been solved (Nagel, 1991). ... however well one works things out top down, there is an ineliminable moral friction where top down meets bottom up. Where policy meets patient, the doctor has moral choices to make which no code of medical ethics can reduce to routine (Hollis, 1988). © 1995 Human Sciences Press, Inc.
