Vascular microsurgery for erectile dysfunction

From 1984 to 2004, 162 deep dorsal vein arterialization were performed in patients suffering from erectile dysfunction EDin spite of non invasive treatment (Phosphodiestherase 5, intracavernous injections of Prostaglandine E1). Two series were evaluated using the International Index of Erectile Function (IIEF, Rosen) before and after surgery. The first serie 1998-2003, 50 patients, mean age : 45 +/- 10 years, 28responders (56%). Comparing the 5 criterias vith a folloup of 4 +/- 2 years, 4 of them get a significative p vlue : erectile function, orgasmic function, intercourse satisfaction, sexual desire has not been significativly improved. For the second serie 1999-2004 we receive 18 responses (64%) in 28 patients oprated on. Mean age : 44 +/- 10years and followup : 7 +/- 2 years. The results were similar to those of the first serie. The most frequently complication was hyperemia of the glans. Taken as a whole we get in 78 cases : satisfaction 70%, improved 12%, failure 18%. Our results are comparable to those of the literature. The DDVA is a reasonable option to treat vascular ED, it respect the cavernous body and blood supply. Does not preclude to use prosthesis implant.
