Frequency and risk factors. The rising numbers of primary joint replacements and revision surgery as well as a changing age structure have led to an increase in intraoperative periprosthetic fractures. Risk factors are revision surgery, uncemented cups, stems and long stems, reduced bone quality and female sex. Preventive procedures have to be taken into consideration. Therapy. The intraoperative diagnosis of this type of fracture is essential for adequate treatment and an excellent outcome without impairment in function or quality of life. The treatment focuses on fracture stabilization, prevention of propagation and on stable fixation of the prosthesis.
