In recent years an increasing interest in research on organ donation and according to statistics the number of people waiting compounded increases while the availability of organs to allow them to regain their health, In this situation it is important to know the attitude and the factors associated with this attitude toward organ donation, there being an instrument for the measurement. Objective: To design and validate an instrument to measure attitudes toward organ donation among relatives of hospitalized patients, Material and Methods: We designed a 24-question questionnaire and analyzed the construct validity through the clarity and relevance of each question and its contents through the test of sample adequacy Kaiser-Meyer-Olkin test of sphericity and Bartlet. A pilot test was conducted by interviewing 78 families of patients in Hospital II Luis Heysen Inchaustegui and I Naylamp Hospital in Chiclayo. We assessed the reliability of the questionnaire analyzing the internal consistency using Cronbach's alpha, Results: Cronbach's alpha of the questionnaire was 0.771. The value of Kaiser-Meyer-Olkin was 0.696 and Bartlett's test of sphericity was less than 0,05, so it was considered appropriate to conduct factor analysis, obtaining the instrument four dimensions: economic, social, cultural, ethical and moral.
