Our Results of Surgical Management in Bronchiectasis

Objective: Bronchiectasis still remains as a serious health problem for our country. Surgery has an important place in its treatment. The aim of this study was to review the outcomes of our treatment method for bronchiectasis. Materials and Methods: In this study, 69 patients (42 male and 27 female) operated with the diagnosis of bronchiectasis between January 2003 and January 2007 in our department were retrospectively analyzed. Results: The mean age of the patients was 21 years (6-62). Computerized tomography was performed on 20 patients and high-resolution spiral tomography, on the others. Broncography was not applied in any of the patients. The indication for operation for all the patients was recurrent infection and prulent phlegm. The disease was predominantly located into the left lower lobe (27 patients, 39.1%). In surgical treatment, 27 patients were performed lower left lobectomy (in 5 patients, additional lingulectomy); 7 patients, pneumonectomy; 3 patients, uppe rleft lobectomy; 2 patients, bilobectomy inferior, 1 case, bilobectomy superior, 6 patients, lower right lobectomy; 6 patients, upper right lobectomy; 8 patients, right middle lobectomy; and 7 patients, only lingulectomy. No mortality was observed. Four patients developed postoperative hemorrhage; 2 patients, bronchopleural fistula, and 3 patients, athelectasy. In the long-term follow-up, all the patients recovered.. Conclusion: Curative resection should be applied in bronchiectasis, that is resistant to medical treatment, especially the saccular type.
